Tangshan cardiological disease institute ,the Department of Cardiology at Tangshan Workers' Hospital of Hebei Medical University is one of the largest cardiological services among the general hospitals of our country. It was initiated in 1980 under the direction of Dr Liu Zhitao, a famous cardiologist in Tangshan city, who was the first director of cardiology department, and is geared to developing under the leadership of present director, Dr.Shang Xiaoming and vice director, Dr.Jiang Yuru. The staff members in the department consist of 30 doctors at various levels, including 6 professors and vice professors and 1 tutor of postgraduate, over 5 of whom have the master degree. There are 100 common beds and 9 monitoring beds. It provides specialized outpatient services on the working days and specialist outpatient service.
Under the direction of our hospital, the department has been developed since initiation, which obtained certain achievement in medical service, teaching and scientific research. At present, the department has 1277 persons/time annual attendance of inpatients and 23570 persons/time annual outpatients number(take the number of 1999 as an example).Since 1970s it has been the first in Hebei province to develop the examination of left and right cardiac catheterization, electrophysiological study of arrhythmia, in vitro electric defibrillation, coronary arteriongraphy, permanent pace-makers of all kinds, intra-aortic balloon counterpulsation, radio-frequency ablation in treating arrhythmia, intravenous implantation of auto pacemaking and conversion of heart rate defibrillation(ICD), intracoronary thrombolysis, PTC,and intracoronary stent implantation.
Since beginning of the department, it has published nearly 100 articles at the foreign and domestic journals and several monographs and text books. It has trained 10 postgraduates and presided and involved in several key programs of scientific research, including clinical study of emergency PTCA-with the monitor of hemodynamics of Swan-Ganz float catheter , Application of mRNA differential display in the clone of angiosmooth muscle cell proliferation-related gene following angioplasty, and the changes of cardiac ventricle late potential in acute myocardial infarction. Several programs have been awarded prizes including the third prizes for progress in science and technology of Hebei Province and the first prize for progress in science and technology as well as the first prize of Tangshan Health bureau for which the department was evaluated as outstanding collective of Hebei province in 1999.It was appointed the key institute of Tangshan health system in 1999, which has a certain impression over the country so it becomes the hot major in the national postgraduate examination enrollment. The intervention therapy has taken a leading position domestically.

Introduction to Director
Shang Xiaoming, chief physician and tutor of doctor degree, is working on the diagnosis and treatment of cardiology disease, scientific research and teaching for many years. He is particularly proficient in interventional diagnosis and therapy of coronary heart disease(PTCA and implantation of stent),cardiac electrophysiology, artificial cardiac pacing, drug treatment in arrhythmia and heart failure. In recent years he has published 20 articles about cardiological diseases and won two third prizes for progress in technology and science of Heber province, two first prizes for progress in sciences and technology of Tangshan city and one second prize for progress in science and technology of Tangshan City. His major research focus is interventional diagnosis and treatment of coronary heart disease, artificial cardiac pacing and cardiac electrophysiology. Since 1982 when the first case of permanent cardicac pacemaker was performed in our hospital, 400 cases have been carried out.500 cases of artificial endocardium temporary pacemaker and 220 cases of radiofrequency ablation were performed in our hospital, through which the diagnosis and treatment of arrhythmia has taken an advanced level in our country.
Dr.Shang Xiaoming has carried out a series of research in severe arrhythemia accompanied with atrial fibrillation and supraventricular tachycardia complicated with refractory heart failure by use of electrophysiological multipoint pacemaking.
ICD has been used to successfully rescue 2 patients of malignant arrhythmia who developed sudden death because of ventricular fibrillation, which is a new approach to treat refractory ventricular fibrillation.
PTCA and Stent implantation have been performed in treating coronary heart disease, electrophysiological examination and radio-frequency ablation in the treatment and diagnosis of arrhythmia, PBMV in treating mitral stenosis in rheumatic heart disease, bedside Swan-ganz right float catheter in monitoring hemodynamics for evaluating the clinical medicine therapeutic effect.

Interventional therapy center of Tangshan Gongren Hospital has recently performed up to 10.000 cases of interventional operation including 1500 cases of coronary arteriongraphy, 380 cases of PTCA and 78 cases of emergency PTCA because of acute myocardial infarction.
The first case successfully treated through PTCA in Switzerland in 1977 opened a new era for the interventional therapy of coronary heart diseases, as an important method to treat ischemic heart disease, which now has been widely applied through the world. In 1955 30.000 patients were treated by PTCA only in America, accumulating to 1 million in 1998.In China, PTCA has been carried out in city-level hospital, up to 10.000 patients annually. With the renew and expand of PTCA itself, a series of complemented technologies, such as coronary endometrium stent, orienting rotary ectomy of coronary artery, atherectomy by laser, and ultrasound coronary angioplasty, has been developed, which provides a wide space for the interventional therapy.In addition, the gradual improvement of interventional devices, especially the application of intracoronary stent has improved the success rate and complications of interventional therapy.
Through the performance of interventional therapy, the diagnosis and treatment of cardiological disease has been greatly improved as well as produced better economic profit. Only in 1999 there were 309 persons receiving interventional therapy because of coronary heart disease, accounting for 12.50% of the yearly operation. Emergency PTCA can maximally rescue survived myocardium, improve cardiac function and prognosis of myocardial infarction, especially suiting the patients with contraindications of thrombolysis and complications, in particular the patients complicated with cardiogenic shock, and lower the mortality rate. Since 1996 when PTCA has been carried out in our hospital, there are 380 cases placed on PTCA, including 78 cases of emergency PTCA. We have accumulated rich experiences in the performance of PTCA and own an army of quick-response skilled physicians. We have planed to explore the therapeutic effect of emergency PTCA through emergency PTCA of AMI, from electrocardiograph,cardiac ultrasound, myocardium ECT, laboratory to clinical therapeutic effect. In particular Swan-Ganz float catheter has been used to monitor the changes of hemodynamics following emergency PTCA, so to accurately evaluate the therapeutic effect of emergency PTCA.

Interventional therapy center of rheumatic heart valve disorders
Since balloon ablation of mitral stenosis caused by rheumatic cardiac valve disorders was carried out in 1996 in our hospital, balloon ablation of mitral stenosis and pulmonary valve stenosis was performed in 120 cases, which relieved patients ' suffering of operation and improved the postoperative living quality.

Cardiology Intensive Care Unit (8-beds): treating approximately 700 inpatients each year, focuses on the management of coronary heart disease, unstable angina pectoris, acute myocardial infarction, cardiogenic shock complicated arrhythmia, difficult heart failure, rheumatic heart valve disorders complicated with heart failure. The unit has experiences in the thrombolysis therapy of acute myocardial infarction within 6 hours, and monitoring of invasive hemodynamics by use of bedside Swan-Ganz right cardiac float catheter and intraaortic balloon counterpulsation.
Currently, under the instruction of chief physician, doctor Liu Zhitao, the cure rate of critical cardiac disease has been greatly improved, accumulating certain experiences in the diagnosis and treatment of critical cardiac disease, the actual mortality rate of acute myocardial infarction is below 10%. The application of Swan-ganz float catheter to the monitor of hemodynamic changes following emergency PTCA contributes great to the evaluation of the therapeutic effect of emergency PTCA and provides the reliable basis for the widely use of emergency PTCA.

NON-invasive examination in cardiology department : regular ultrasonic cardiogram, transesophageal echocardiography, loading ultrasonic cardiogram, myocardium ECT, flat exercise test, dynamic blood pressure, and dynamic electrocardiogram facilitate the diagnosis of cardiac disease and the evaluation of the prognosis of cardiac disease.

Laboratory and scientific programs
The restenosis rate after PTCA challenges the development of interventional therapy, because vascular restenosis after interventional therapy is still one of the difficulties unresolvable in the world, which is the international hot topic both from clinical to basic medicine and from technology to medicine and gene therapy. At present the focus of the study of restenosis lays in the relationship between restenosis and the changes of one gene or one factor, such as multiple vasoactive substances and growth factors, but the limitation of these methods is recognizable. Now the staff plans to fully understand which gene ' s expression is involved in the proliferation of restenosis smooth muscle cell through the study of the different phases of primary cell proliferation of smooth muscle cell after angioplasty, which greatly contributes to the clarification of molecular mechanism of restenosis and gene therapy of restenosis.
Fortunately the clone of vascular smooth muscle cell proliferation-related gene by mRNA differential display provides the clinical basis and theory for the gene therapy and prevention of restenosis following angioplasty.

In order to improve the treatment level of cardiological diseases and popularize the interventional therapy and technology of cardiological disease, Tangshan health bureau, together with Beijing Fuwai Hospitan, Beijing Medical University, Beijing Anzhen Hospital, Tangshan Gongren Hospital and Tangshan Medical Association, held the 2000 demonstration of interventional therapy and progress in cardiological disease treatment, for which famous cardiologists were invited to give lectures and make operational demonstration.

Intracoronary intima circumflex was first successfully carried out in 1999 in our province, Cardiology department of Tangshan Gongren Hospital conducted successful coronary artery aplasty and stent implantation by application of double lead catheter and bilateral coronary arteriongraphy, with the help of Japanese director of Cangfu Central Hospital.Thus, the success rate of recatheterization of occluded coronary artery has been greatly increased and the formation of pseudocavity was avoided to some extent, meanwhile the pain because of pass-b for occluded coronary artery was relieved and paved the way for the application of PTCA. |
